Three people were killed and another injured during lifeboat maintenance work on a floating storage and offloading vessel operated by Malaysia’s state energy company Petronas, the firm said on Monday.

The accident occurred at around 12:50 p.m. local time on Sunday aboard the FSO Sepat vessel off the coast of Malaysia’s Terengganu state. Petronas said three workers were declared dead upon arrival at a hospital, while a fourth worker was evacuated for treatment and remains under observation.

The company said an investigation into the incident is underway in coordination with relevant authorities but did not provide further details.

According to Malaysia’s state news agency Bernama, the workers had boarded a lifeboat to carry out maintenance operations at sea. Local police believe the rope or hook attached to the lifeboat may have detached, causing the workers to fall into the water.
